Default I do not think so.

I too read about such complaints but most of these look excuses to me for getting banned.

1."Lurkers click on the ads to go away from your bad site, but you make money for this"

It might just happen but what are the odds? Leave alone the odds for clicking but odds for visiting a bad site?

2. "At times, the unaware users click on ads
Again how many times it may happen. And these are not fraudulent clicks for sure.

So forget about those negative feedbacks, concentrate on getting visitors first.
